I, {name}, acknowledge that participation in martial arts training and related activities involves inherent risks, including injury, disability, or death, and I voluntarily assume all such risks. I release and hold harmless East Cloud Kungfu LLC, its owners, instructors, staff, and affiliates from any and all liability or claims arising from participation, including those due to negligence. I certify that I/my child am physically able to participate, agree to follow all rules and instructions, and understand that the school is not responsible for lost or stolen property. I grant permission for East Cloud Kungfu LLC to use photos or videos of me/my child for promotional and educational purposes without compensation. If signing for a minor, I confirm I am the parent/legal guardian and agree to these terms on their behalf. I have read and understand this agreement and sign it voluntarily.
